Welcome Letter
My staff and I thank you for choosing to consult with us. To ensure that you
obtain the maximum benefit during your time with us, please review this material
and write down any questions. Completing the registration packet and mailing
or emailing it back to us as soon as possible, will streamline your consultation.
If
you wish to discuss more than one area of interest, please notify our office
ahead of your appointment so that an adequate amount of time can be allocated
for your visit. Please arrive at our office 10 to 15 minutes before your scheduled
appointment time, to complete any health information forms.
During your visit,
you will meet with me and my staff. You will have a chance to speak with our
clinical assistants. Our staff will discuss professional fees and scheduling
options with you. Photos may be taken if you decide to schedule surgery. Facial
photographs will require removal of your makeup and you may wish to bring some
makeup with you if you would like to reapply it prior to leaving office.
To
ensure the safety of your surgery, medical clearance from your personal physician
may be deemed necessary prior to surgery. This will be discussed with you during
your consultation. Following any surgery that you may have, it may be necessary
for you to remain in the city for a period of time. Please clarify this with
me or my staff to avoid conflicts with your other activities or travel plans.
Full
payment for the consultation (if cosmetic) or a co-payment (if through insurance)
will be collected at the time of your visit. We accept Visa and MasterCard.
Please note that we require 2 days notice prior to canceling an appointment,
to avoid any cancellation fees.
